From the moment we’re contacted by a patient’s physician, we begin coordinating a complete home health care program, working closely with the hospital, physician, patient, family and insurance company.

PROFESSIONAL SKILLED NURSING CARE

Registered Nurses, working under a physician’s orders, provide care and instruction to patients and their families.  Services include but are not limited to:

  • injections
  • IV therapy
  • maintenance of urinary catheters
  • venipuncture
  • dressing change and instruction
  • ostomy care
  • diabetic care and instruction


 HOME HEALTH AIDES

Certified Home Health Aides, working under the supervision of Registered Nurses, are trained to provide assistance in:

  • dressing
  • bathing
  • personal grooming
  • light housekeeping
  • prescribed exercise
  • bowel and bladder training
  • other treatments as directed

Aide services may be provided when patients require visits by a Registered Nurse or Physical or Occupational Therapist.


SOCIAL SERVICES

Services are provided by licensed Medical Social Workers:

  • grief counseling
  • coping skills for social, emotional and financial problems
  • coordination of community resources


OCCUPATIONAL, PHYSICAL AND SPEECH THERAPY 

Licensed therapists provide rehabilitation programs for home independence:

  • functional daily living skills
  • improve strength and endurance
  • therapy for speech and swallowing
  • home safety evaluation
  • fall prevention
