North Hawaii Community Hospital is proud to offer Diabetes Wellness services to people in our community. Our Diabetes Wellness Clinic is led by a Registered Nurse (RN) who is also a Certified Diabetes Educator (CDE) with over 12 years serving our local community.
Outpatient Education Program
We offer a comprehensive program for the patient who is newly diagnosed or just needs a refresher. Topics presented include: overview of diabetes acute and chronic complications home blood-glucose monitoring medications hyper and hypoglycemia nutrition stress management exercise In addition, one-on-one consultations with our RN, CDE are available to patients for a more individualized approach.
Gestational Diabetes Program
Our Gestational Diabetes Program is designed to help expectant mothers diagnosed with gestational diabetes learn how to eat properly and monitor their blood sugars. Our RN, CDE will remain in close contact with the patient and patient’s family throughout their pregnancy and after care.
Outpatient Insulin-pump Program
This highly effective therapy may significantly reduce the onset and progression of complications associated with diabetes. After a thorough assessment to determine if the patient is an appropriate candidate for the insulin pump, our RN, CDE will schedule with the patient to provide intensive “pre-pump” training. This is followed by an intensive one-day insulin pump “start up” that is coordinated with the team, including the physician. The success rate of this program has been outstanding.
Continuous Blood-Glucose Monitoring
Our nurse-clinician is certified to provide this specialized service, utilizing the continuous blood-glucose monitor.
Through a special infusion set attached to the patient, this small device monitors a patient’s blood sugars every 5 minutes for a continuous 72-hour period. This information is then printed out and forwarded to the physician to assist in his or her individualized plan of care.
